We’re excited to share it with you, as it highlights the incredible progress being made to improve health equity, expand access to care, and strengthen communities across Iowa.

👩⚕️ Meet Ernesta, the New HUB Lead Navigator
With two decades of experience in Human Services, Ernesta brings deep knowledge and a passion for health advocacy to her leadership role.
🧩 Welcoming New Board Members
The HUB has added four dedicated professionals whose expertise spans nursing leadership, multicultural outreach, public health, and equity-centered community development.
🥦 HEAL Project Success
Through the Healthy Eating and Active Living (HEAL) initiative, the HUB and its partners—including Grace Fitness—are creating sustainable systems change in the 50314 area to support healthier lifestyles.
✅ Highlights include:
69 participants supported through wellness programs
167 produce boxes delivered
A successful site visit from USDA and Iowa HHS
🏥 Recognized by the CDC
The CDC has selected the HUB for a national Evaluability Assessment, spotlighting their innovative approach to lifestyle change and community health improvement.
🍅 Fighting Food Insecurity Statewide
Through partnerships and grant support, over 1,248 produce boxes have been distributed to nearly 370 Iowans through the Community Food Project.
🚶♂️ Spring into Summer with MOVE YOUR WAY
In partnership with ISU and Healthy Iowans, the HUB is helping expand access to the Walk with Ease program—now open to all adults across Iowa.
🧠 Empowering Youth with APEX
High school students from Waukee APEX teamed up with the HUB to present an anti-bullying campaign, reaching dozens of elementary students and sparking important conversations.
💡 Design Sprint Innovation
A joint initiative with Primary Health Care and the National Association of Chronic Disease Directors is enhancing arthritis care by connecting patients with community resources.
📢 Voices for Equity in Nursing
Board Chair Dr. Jimmy Reyes co-authored a powerful article on inclusive nursing practice—read it here.
📊 2024 in Review
The HUB celebrated a 221% increase in grants and contracts, expanded programming, and deepened its impact statewide. Over 3,500 individuals were served through 225 partner-led workshops!
